Document Requirements
To understand which documents are required to prove dependent eligibility, please review each document listed next to the dependent(s) relationship on your Documentation Checklist mailed to you by Consova. The additional information below describes what documentation will be considered valid for the purpose of this verification.
Spouse
Plan Definition
- Your legally married Spouse
Legally separated or divorced spouses are not eligible for coverage
Document Requirements
A copy of marriage certificate.
AND
If your marriage did not occur in the current calendar year, please also submit a copy of page one through the signature line of your Jointly filed Federal Tax return form 1040 (or Puerto Rico Tax return) or both of your tax returns if you file separately, from the most recent Tax season. Please black out the first five digits of any Social Security Numbers and any IRS Identity Protection PIN's. If you do not file as married (either filing jointly or separately), please contact Consova for alternative documentation requirements.

NOTE: You only need to complete this form if enclosed in the letter you received from Consova. Consova will reach out to the employer contact listed on the form to verify benefits eligibility.
Domestic Partner
Plan Definition
- Your eligible, unrelated domestic partner
Document Requirements
Copy of the Domestic Partner Affidavit (signed by you and your partner)
AND
Proof of dependency as evidenced by a copy of two of the following documents:
- Proof of shared residence via joint mortgage statement or rental agreement
- Automobile title or registration showing joint ownership of vehicle
- Joint checking, bank or investment account statement
- Joint credit account statement
- Utility Bill for shared residence with both the associate's name and partner’s name
- A Will or life insurance policy showing partner as beneficiary
- Automobile insurance bill showing both associate's and partner's name
Please note: Proof of dependency documents need to be dated prior to the date of this letter from Consova. One document must be dated 6 months prior to the date of the letter and the second document must be dated within 60 days prior to the date of the letter. Child under age 26
Plan Definition
- Your natural child, legally adopted child, or child in the process of being adopted;
- Stepchild;
- A child of your Domestic Partner; or
- A child whom you have legal guardianship of; or
- A grandchild, as long as the associate’s covered dependent, who is the parent of the grandchild, qualifies as a tax dependent of the associate or the grandchild qualifies as a tax dependent of the associate, as determined by the IRS.
- A child who is the subject of a Qualified Medical Child Support Order (QMCSO) issued to you
Document Requirements
A copy of the following documents (varies by the relationship of the child to the Employee):
- Natural child, legally adopted child, or child in the process of being adopted: State or county issued birth certificate showing employee’s name or signed court order. If birth certificate lists employee’s maiden name or a different name than in Humana's records, please provide documentation of the name change (marriage certificate, divorce decree or other court documentation of the name change).
- Stepchild: State or county issued birth certificate showing parents’ names, copy of your marriage certificate, and copy of your Jointly filed Federal Tax return (or Puerto Rico Tax return) from the most recent Tax season (front page only).
- Child of your domestic partner: State or county issued birth certificate and proof of established Domestic Partnership.
- Child whom you have legal guardianship: Signed court order and your Federal Tax return (or Puerto Rico Tax return) from the most recent Tax season claiming the child as a dependent.
- Grandchild: State or county issued birth certificate showing parents’ names for child (parent of the grandchild) and State or county issued birth certificate showing parents' names for the grandchild and your Federal Tax return (or Puerto Rico Tax return) from the most recent Tax season claiming the child and/or grandchild.
- Child who is the subject of a Qualified Medical Child Support Order: Signed court order.
Please note: for unmarried disabled children aged 26 or over, in addition to the birth certificate, we will also need a copy of your Federal Tax return (or Puerto Rico Tax return) from the most recent Tax season claiming the child (front page only).
Extended Family Member
Plan Definition
One adult who meets all of the following qualifications:
- Between the ages of 18 and 65, and
- Qualifies as the associate's IRS Tax dependent, and
- Related to the associate as a child, grandchild, sibling or niece/nephew, and
- Has lived with the associate for at least six consecutive months and plans to remain a member of his/her household during the period of coverage, and
- Not be of a previous generation (no parents, grandparents or aunts/uncles)
Document Requirements
A copy of the following documents (varies by the relationship of the dependent to the Associate):
- A copy of the Extended Family Member Affidavit.
AND
- A copy of your Federal Tax return (or Puerto Rico Tax return) from the most recent Tax season (front page only); please black out the first five digits of your SSN and all financial information, claiming your extended family member as a dependent.
AND
- Natural child, legally adopted child, or child in the process of being adopted: Birth certificate showing associate’s name or signed court order.
- Stepchild age 26 or over: Birth certificate and your marriage certificate.
- Grandchild: Birth certificates for child and grandchild.
- Great grandchild: Birth certificates of great-grandchild, your grandchild and parent of grandchild.
- Daughter or Son-in-Law: Birth certificates for your child and your child’s marriage certificate.
- Sister or Brother-in-Law: Birth certificates of spouse and your spouse’s sister or brother and your marriage certificates.
- Brother, Sister, Half-Brother, or Half-Sister: Birth certificates for you and your brother/sister/half-brother/half-sister.
- Stepbrother or Stepsister: Birth certificates for you and your stepbrother or stepsister and your parents’ marriage certificate.
- Your blood-related Niece or Nephew: Birth certificates for you, your sister or brother (the parent of your niece or nephew), and your niece or nephew.
