Document Requirements
To understand which documents are required to prove dependent eligibility, please review each document listed next to the dependent(s) relationship on the communications mailed to you by Consova. The additional information below describes what documentation will be considered valid for the purpose of this verification.
Spouse
Plan Definition
Your legally married spouse
• Legally separated or divorced spouses are not eligible for coverage
Document Requirements
A copy of your marriage certificate*
AND one of the choices below:
A copy of page one through the claimed dependents section of your jointly filed federal tax return (form 1040) or both of your tax returns if you file separately, from the most recent tax season. Please black out the first five digits of any Social Security numbers.
OR
A Joint Financial Statement dated 2/22/2024 - 4/21/2024 (i.e. joint bank account or joint mortgage statement). If joint documents are not available, individual statements from the Associate and Spouse indicating the same residence will be accepted. Please black out financial information and account numbers.
* If you were married on or after 1/1/2024, only the marriage certificate will be required.
Domestic Partner
Plan Definition
Your domestic partner or "partner" is eligible for benefits if you and your domestic partner:
• Are not married to each other or anyone else,
• Meet the age for marriage in your home state and are mentally competent to consent to contract,
• Are not related in a manner that would bar a legal marriage in the state in which you live, and
• Are not in the relationship solely for the purpose of obtaining benefits coverage.
Alternatively, any other person to whom you are joined in a legal relationship recognized as creating some or all of the rights of marriage in the state or country in which the relationship was created is also a domestic partner.
Document Requirements
A copy of your Ingram Domestic Partner Certification Form (signed by you and your partner) or Domestic Partner Registration. Consova will accept a previously completed affidavit or a newly completed affidavit.
AND
One (1) of the following documents as proof of your relationship:
• Proof of shared residence via joint mortgage statement or rental agreement
• Joint checking, bank or investment account statement*
• Joint credit account statement*
• Joint utility bill*
• A will and/or life insurance policy which designates the other as primary beneficiary
* Please note: These documents need to be dated between 2/22/2024 - 4/21/2024. If joint documents are not available, individual documents from associate and partner indicating the same residence will be accepted. Please black out financial information and account number.
Child under age 26
Plan Definition
• Your natural child, legally adopted child, or child in the process of being adopted;
• Stepchild;
• A child of your eligible Domestic Partner;
• A child whom you have legal guardianship of; or
• A child who is the subject of a Qualified Medical Child Support Order (QMCSO) issued to you. (If you have a QMCSO on file with Ingram, your dependent is not included in the verification.)
Document Requirements
A copy of the following documents (varies by the relationship of the child to the Associate):
• Natural child or legally adopted child: State or county issued birth certificate showing associate’s name or signed court order. If birth certificate lists associate’s maiden name, please provide marriage certificate.
• Stepchild: State or county issued birth certificate showing parents’ names, copy of your marriage certificate, and a copy of page one through the claimed dependents section of your jointly filed federal tax return (form 1040) or both of your tax returns if you file separately, from the most recent tax season OR A Joint Financial Statement dated 2/22/2024 - 4/21/2024 (i.e. joint bank account or joint mortgage statement). If joint documents are not available, individual statements from the Associate and Spouse indicating the same residence will be accepted. Please black out the first five digits of any Social Security numbers and any financial information or account numbers.
• Child of your domestic partner: State or county issued birth certificate and proof of established Domestic Partnership.
• Child of whom you have legal guardianship: Signed Court Order and a copy of page one through the claimed dependents section of your filed federal tax return (form 1040) from the most recent tax season claiming the child as a dependent. Please black out the first five digits of any Social Security numbers.
• Child who is the subject of a Qualified Medical Child Support Order: Signed court order.
Please note: For unmarried disabled children aged 26 or over, in addition to the birth certificate, we will also need a copy of page one through the signature line of your filed federal tax return (form 1040) from the most recent tax season claiming the child as a dependent. Please black out the first five digits of any Social Security numbers.
